Dear Ms. Thomashauer:

We are writing to ask your assistance in correcting a deficiency in your CAQH Universal Application.  Despite being licensed in 50 states and the District of Columbia, licensed professional counselors do not have their license designation listed as a choice on the application/reattest form.  Under the section “Professional IDs,” there is a field for entering information on one’s license (Medical License). Specifically, under License Type, there is a dropbox that lists virtually every other possible license type except ours, which would be LPC and also LMHC.  (While we are one profession – counseling, we are sometimes licensed as “licensed professional counselor” and sometimes as “licensed mental health counselor,” depending on the state title).

We are hoping you can assist in remedying this situation. On behalf of licensed professional counselors across the nation, we appreciate your consideration of our request.
