Dear Ms. Thomashauer: We are writing to ask your assistance in correcting a deficiency in your CAQH Universal Application. Despite being licensed in 50 states and the District of Columbia, licensed professional counselors do not have their license designation listed as a choice on the application/reattest form. Under the section “Professional IDs,” there is a field for entering information on one’s license (Medical License). Specifically, under License Type, there is a dropbox that lists virtually every other possible license type except ours, which would be LPC and also LMHC. (While we are one profession – counseling, we are sometimes licensed as “licensed professional counselor” and sometimes as “licensed mental health counselor,” depending on the state title).
